Air India Pilot Tests Positive for Cannabis After Fatal Crash

Post-mortem toxicology reveals THC but cannot determine impairment at time of incident that killed 191 passengers.

An Air India pilot involved in a fatal August 2026 crash tested positive for cannabis metabolites during post-mortem toxicology screening, according to India's Directorate General of Civil Aviation, but the test cannot establish whether the pilot was impaired during the flight or when consumption occurred.

Post-Mortem Toxicology Confirms THC Presence

India's Directorate General of Civil Aviation confirmed Friday that toxicology analysis detected THC metabolites in tissue samples from the deceased pilot. The Boeing 737 crashed on approach to Calicut International Airport on August 12, killing all 191 passengers and crew. DGCA spokesman Arun Kumar named Captain Deepak Sathe as the pilot who tested positive, though Kumar declined to specify THC concentration levels or the testing laboratory used.

The agency collected samples within six hours of the crash. Standard aviation post-mortem protocols in India test for alcohol, cannabinoids, opiates, amphetamines, and benzodiazepines.

Detection Window Spans Weeks, Not Hours

THC metabolites remain detectable in human tissue for 3 to 30 days after consumption, depending on frequency of use and individual metabolism. That makes it impossible to determine proximity of use to the crash. Dr. Barry Sample, senior science consultant at Quest Diagnostics and former president of the International Association of Workplace Drug Testing Programs, told the BBC that a positive post-mortem cannabis test "tells you nothing about impairment at the time of the incident."

Key limitations of post-mortem cannabis testing include:

  • No correlation between metabolite concentration and cognitive impairment
  • Detection windows ranging from days to weeks after last use
  • Post-mortem redistribution that can artificially elevate THC levels in tissue
  • Inability to distinguish between THC from medical cannabis, hemp products, or recreational use

India Maintains Zero-Tolerance Aviation Policy

Indian aviation regulations prohibit pilots from operating aircraft within 12 hours of consuming alcohol or while under the influence of any psychoactive substance. The DGCA's Civil Aviation Requirements (CAR) Section 3, Series M, Part II mandates random drug testing for flight crew and defines cannabis as a prohibited substance regardless of legality under state law. India's Narcotic Drugs and Psychotropic Substances Act of 1985 criminalizes cannabis possession and consumption nationwide, though enforcement varies by state.

Air India's employee handbook requires pilots to submit to random drug screening. Any positive test result triggers immediate suspension. The airline conducts approximately 2,400 random drug tests annually across its pilot workforce of 1,600, according to 2025 company disclosures.

Investigators Focus on Flight Data Recorder

The Aircraft Accident Investigation Bureau recovered both the cockpit voice recorder and flight data recorder intact and has prioritized analysis of the final 90 seconds of flight. Preliminary data reviewed by the AAIB shows the aircraft descended below the glide slope at 1,200 feet and failed to execute a go-around despite ground-proximity warnings, according to a source familiar with the investigation who requested anonymity because the probe is ongoing.

The AAIB hasn't attributed the crash to pilot impairment. Investigators continue to examine weather conditions, runway surface contamination, and potential mechanical failure. Calicut experienced heavy monsoon rain at the time of the incident, and the airport's tabletop runway design has been cited in three prior fatal crashes since 2010.

Workplace Cannabis Testing Under Global Scrutiny

Aviation regulators in 38 countries now face pressure to adopt impairment-based testing protocols as cannabis legalization expands. No commercially viable real-time impairment test exists. The International Civil Aviation Organization published guidance in March 2026 recommending member states transition from metabolite-detection urine tests to oral-fluid testing, which narrows the detection window to 12-24 hours but still can't measure active impairment.

The U.S. Federal Aviation Administration maintains a lifetime ban on pilots who test positive for THC, regardless of state medical-cannabis laws. Canada's Civil Aviation Directorate prohibits cannabis use within 28 days of flight duty, the longest abstinence window among G7 nations.

Final Report Expected in 18 Months

The AAIB will release a preliminary factual report within 30 days and expects to publish final probable-cause findings by February 2028. The agency has interviewed surviving ground crew, reviewed Air India's pilot-training records for Captain Sathe, and requested maintenance logs for the aircraft, a 2019-vintage 737-800.

Toxicology results will be included in the final report but are unlikely to be cited as a contributing factor without corroborating evidence of impairment from cockpit voice recordings or witness testimony, according to retired AAIB investigator Mohan Ranganathan, now an aviation-safety consultant in New Delhi.

The crash marks India's deadliest aviation disaster since 2010. That year, an Air India Express 737 overran the same Calicut runway, killing 158. Investigators cited pilot fatigue and failure to execute a go-around as primary causes.
